Overview

Teriparatide is a recombinant fragment of human parathyroid hormone comprising its first 34 amino acids (PTH 1-34). It is an anabolic, or bone-building, agent used in the treatment of osteoporosis, and is marketed under brand names including Forteo and Forsteo.

Unlike most osteoporosis medications, which primarily slow the breakdown of bone, teriparatide is studied for its ability to actively stimulate new bone formation. It is FDA-approved and prescribed under clinical supervision, typically for patients at high risk of fracture.

Its approved indications include postmenopausal women with osteoporosis at high fracture risk, men with primary or hypogonadal osteoporosis, and people with osteoporosis associated with long-term glucocorticoid (steroid) therapy.

How it works

Parathyroid hormone regulates calcium balance and bone remodeling. A key finding in the research is that the pattern of exposure matters: intermittent, once-daily dosing of teriparatide is reported to favor bone formation, whereas continuous elevation of parathyroid hormone tends to favor bone resorption.

In studies, teriparatide is described as acting on the parathyroid hormone receptor to increase the activity and number of bone-forming cells (osteoblasts). Researchers have linked this to effects on growth factors such as IGF-1 and on signaling molecules like sclerostin, contributing to increased bone mineral density and improvements in bone structure.

Reported benefits

  • Studied for stimulating new bone formation, rather than only slowing bone loss
  • Reported to increase bone mineral density in the spine and hip
  • Studied for reducing the risk of vertebral and nonvertebral fractures in high-risk patients
  • Investigated off-label for applications such as fracture and spinal-fusion healing (evidence still emerging)

These are reported and studied effects observed in clinical settings, not guaranteed outcomes for any individual.

Considerations & side effects

Commonly reported side effects include nausea, dizziness or lightheadedness, leg cramps, and elevated blood calcium. Because it acts on calcium metabolism, teriparatide is generally avoided in people with pre-existing hypercalcemia or certain other bone conditions.

Regulatory labeling has historically included cautions related to osteosarcoma observed in animal studies, which has informed limits on total lifetime treatment duration. Teriparatide is a prescription medication and should be evaluated, initiated, and monitored by a qualified clinician; this page is educational and not a substitute for medical advice.

Frequently asked

What is teriparatide?

A recombinant fragment of human parathyroid hormone made up of the first 34 amino acids (PTH 1-34). It is an anabolic, or bone-building, agent used to treat osteoporosis, marketed under brand names such as Forteo and Forsteo.

Is teriparatide FDA-approved?

Yes. Unlike most research peptides, teriparatide is an FDA-approved prescription medication, indicated for osteoporosis in postmenopausal women, men, and people on long-term glucocorticoid therapy who are at high risk of fracture.

How is teriparatide different from most osteoporosis drugs?

Many osteoporosis medications work by slowing the breakdown of bone. Teriparatide is different in that it is studied for actively stimulating new bone formation, an anabolic mechanism.

Who is teriparatide typically prescribed for?

It is commonly reserved for patients at high fracture risk, or those who have not tolerated or responded to other osteoporosis therapies. It is a prescription-only medication used under clinical supervision.
